That material exists because those are the phases where the documented fatalities cluster, and it deserves the attention it gets.
What receives far less attention is the period after discharge. Yet this window carries its own distinct risks — some pharmacological, some behavioral — and they are risks that clinic monitoring, by definition, no longer covers. Nobody is watching your telemetry in seat 14C.

Why the Risk Window Outlasts the Experience

The Single Largest Post-Treatment Danger: Collapsed Opioid Tolerance
If there is one item on this list that deserves to be in bold, it is this one.
Ibogaine is used in opioid dependence largely because it can interrupt withdrawal and substantially reduce craving. That interruption also means the person leaving treatment no longer has the opioid tolerance they arrived with. Tolerance falls sharply after any period of abstinence, and it falls fast.

The risk is amplified in the current drug supply. With fentanyl and its analogues dominating the illicit opioid market, the margin for error on any post-abstinence use is far narrower than it was a decade ago.
What reduces this risk:
- Naloxone, on hand and known about. The person leaving treatment should have it, and so should whoever they are living with. Someone else has to be able to use it, because you cannot administer it to yourself.
- Not being alone during the first weeks. This is the practical argument for structured aftercare rather than a sentimental one.
- Honest planning for the possibility of a return to use. Treating relapse as unthinkable does not make it less likely; it makes it more dangerous, because nobody prepared for it. Ataxia, Falls, and the Unglamorous Injuries
Ibogaine reliably produces ataxia — impaired balance and coordination — and it does not resolve the moment the visionary phase ends. Unsteadiness commonly persists for a day or more, and in some people longer.

When Is It Actually Safe to Fly Home?

Consider what air travel demands: navigating a terminal while ataxic, sitting immobile for hours, dehydration, disrupted sleep, and being hours away from medical care in a cabin at reduced pressure — all potentially while an active metabolite is still circulating and cardiac parameters may not have returned to baseline.
There is no universal number of days that applies to everyone, and any source that gives you one without knowing your cardiac history, dose, and recovery trajectory is guessing. The right approach is to have the treating clinician clear you for travel based on your actual condition, and to book flexible return travel so that clearance drives the date rather than the ticket driving the clearance. Building a buffer of extra days into the trip costs very little and removes the pressure to leave before you should.
Restarting Medications — Carefully, and Not on Your Own
Many people taper off psychiatric or other medications before treatment because of interaction and cardiac-conduction concerns. The mirror-image question — when and how to restart — gets far less structured attention, and it is where people improvise.
Two points matter. First, the same interaction concerns that justified the taper do not disappear at discharge, because noribogaine is still present. Sleep disruption. Difficulty sleeping for several days after dosing is frequently reported. It is usually self-limiting. - Who clears me for air travel, and on what criteria?
- What is the written plan for restarting my medications, and which prescriber owns it?
- Is naloxone provided at discharge, and is my family shown how to use it?
- What follow-up contact exists at one week, one month, and three months — and is it included?
- What happens if I destabilize after I get home, at 2 a.m., in a different time zone?

Ibogaine safety is usually framed as a screening problem, and screening genuinely is where the most acute risks are managed. But the treatment does not end when the experience does. An active metabolite is still working, coordination is still impaired, cardiac parameters may not be back to baseline, and — most importantly — opioid tolerance is gone while the drug supply has not become any more forgiving.
